Download the form and fill out both sides accurately. For activities, state “CPP Tennis Club practices and events”. For activity dates and times, put “Fall 2024 CPP Tennis Club scheduled practices and events”. For locations, put “CPP Tennis Courts”. When completed, email to the following email:
